Please find on the link above referral forms for GP Practices to complete when referring patients for procedures subject to a Clinical Threshold policy. The forms should accompany the usual referral letter to the hospital and have been designed for clinicians to sign to confirm the patient meets the referral criteria. The forms are available in both an EMIS and SystmOne format.
The process for dealing with those of your patients who may require referral for any of the procedures subject to a Clinical Threshold policy is:
- Identify that there is an associated Clinical Threshold policy (all are available on Knowledge Management)
- Complete the proforma marking those criteria appropriate to your patient.
- Attach and send the proforma and the referral letter via the NHS e-Referral system.
Referring in line with Clinical Threshold policies is part of the NHS Norwich CCG Demand Management Incentive Scheme. Ensuring that your patients meet the threshold criteria prior to referral will avoid unnecessary appointments at the hospital. NNUH, QEHKL and JPUH would be expected to reject a referral where a clinical threshold applies but this may not be until after the first Outpatient appointment, so getting the referral right first time also means patients expectations are managed appropriately.
